Recalls: Pacifier holders, rattles and more

Infant pacifier holders and rattles sold at Hobby Lobby are being recalled because they pose a choking hazard. Other recalled consumer products include faulty bedside infant sleepers and baby carriers.
Infant Pacifier Holders
Details: Little Wishes Summer Plush Pacifier Holder, including the Blue Whale, item number 5202601, and the Pink Octopus, item number 5725213. They were sold at Hobby Lobby stores nationwide from January through April. The pacifier holders are made of a plush fabric, stuffed with fiber, measure about 4.5 inches by 2 inches and have a 10-inch ribbon attached to the body. The item number is printed on the top left corner of the product hang tag. “Hobby Lobby 9123069” is printed on the front and back of the sewn-in label.
Why: The fin of the whale and the head of the octopus can detach from the pacifier holder, posing a choking hazard.

Infant Rattles
Details: Little Wishes Chenille Stuffed Rattles, including the Pink & Green Fish rattles, item number 5141577, and the Blue & Yellow Fish rattles, item number 5127642. They were sold at Hobby Lobby stores nationwide from January through April. The rattles are made of a soft chenille fabric with a fiber stuffing. They are 8.5 inches by 7 inches and have a hole cut out in the middle. The item number is printed on the top left corner of the product hang tag. Rattles have a sewn-in label with “Reg. No. PA-15130(CN)” and “Hobby Lobby 9123069” printed on the front of the label.
Why: The rattle seams can separate, exposing the fiber stuffing and bell rattle, posing a choking hazard.

Bedside Infant Sleepers
Details: Bednest bedside infant sleepers made after July 2014 and sold online at www.Bednest.com. The date of manufacture is stamped underneath the base of the bedside sleepers. There are sewn-in Bednest labels on the fabric side panel and on the upper surface of the mattress.
Why: They fail to meet mandatory federal requirements for these products, posing a fall hazard to babies

Baby Carriers
Details: The Twin Go Original Carrier sold at boutique stores nationwide and online at Amazon.com and TwinGoCarrier.com from December 2014 through May 2016. It is an infant carrier designed to carry one or two babies against the caregiver’s body at the waist. It is intended for babies who are from 10 to 45 pounds. “Twin Go” is printed on a label located on the front of the carrier and an 11-digit batch number is printed on a label sewn to the waistband. Batch numbers included in this recall: 23014024003, 01515026003, and 21615019001. They were sold in fabric that is black on the outside and blue or orange on the inside.
Why: The waist buckle can break, posing a fall hazard to children in the carrier.

Grinders
Details: Bosch 1380 Slim 4.5-inch angle grinders with date codes 502 through 511. The model number and date codes are located on the name plate affixed to the underside of the grinder. The grinders are blue and silver with a black label and black and red control buttons. “BOSCH” is printed in red on the side of the product. They were sold at Lowe’s, Menards, The Home Depot and other home improvement and hardware stores nationwide as well as online at Amazon.com, HomeDepot.com, Lowes.com and Menards.com from March 2015 through November 2015.
Why: The grinder can overheat while in use, causing the brush covers to melt and expose the end of the brush holder, posing a risk of burns to the consumer.

Weight-Lifting Benches
Details: Olympic Decline free-weight bench with model numbers 16060, 16061, 16062 and 16063 sold via Cybex direct sales force or its distributors directly to gyms nationwide from December 2008 through September 2015. A decal bearing the unit’s model and serial number is affixed to the tube near where the user’s feet are placed when using the unit. The recalled benches have serial numbers C1208 through K0915, which represent manufacture dates of December 2008 through September 2015. “Cybex” is printed on the benches.
Why: The frame on the bench can collapse forward onto the user, posing an impact injury hazard.

Off-Highway Vehicles
Details: Model year 2016 Honda Pioneer 1000 side-by-side recreational off-highway vehicles sold at authorized Honda Powersports dealers nationwide from February through April. The ROVs are motorized off-road vehicles with a steering wheel, a gas pedal, a brake pedal, bucket or bench seats, seat belts and an occupant protection structure. The recalled ROVs were sold in three-seat and five-seat models in red, silver or camouflage color. “HONDA” is printed on the front grill and rear tail gate. “Pioneer 1000” is printed on the sides of the vehicle near the tailgate. The model and serial numbers are on a certification label located at the top rear of the left front wheel well. Why: The front door can open unexpectedly while the vehicle is in use, posing a risk of injury to occupants.

Utility Vehicles
Details: Model year 2013, 2014 and 2015 Kubota RTV400 and RTV500 series gas-powered utility vehicles. Recalled utility vehicles come in orange and camouflage and have serial numbers ranging from 10001 to 22042 for the RTV400 and 25083 to 40991 for the RTV500. The model is on the side of the unit and on the data plate on the back of the cab, between the cab and the bed. The serial number is also on the data plate. They were sold at authorized Kubota dealers nationwide from 2010 to 2015 for between.
Why: The steering shaft can break and cause the operator to lose control of the vehicle, posing an injury hazard due to risk of collision.
