MCHS volleyball tryouts set

Tryouts for sophomore, junior and senior members on the 2016 Morgan City High School volleyball team will be held May 9-11.
The tryouts will be held from 3 p.m. until 5:30 p.m. each day.
Tryouts for incoming freshmen will be held May 12 from 3:15 p.m. to 5:30 p.m.
Those wishing to attend must furnish a copy of a current physical before participating. Physicals expire the last day of the month they were taken in 2015.
Paperwork may be picked up in the front office of Morgan City High School or Morgan City Junior High.
All paperwork must be completed and submitted prior to the first day of tryouts.
Paperwork necessary includes:
—Contract regarding student eligibility (incoming ninth graders).
—Current physical.
—Copy of birth certificate.
—Louisiana High School Athletic Association participation/parental permission form.
—LHSAA substance abuse/misuse form.
—LHSAA concussion statement.
—Bourgeois Medical form.
—St. Mary Consent form.
